Dear Parents & Guardians,
Bath County Health Department will be providing Influenza Vaccines at Valley Elementary, Bath County High School and Millboro Elementary on Tuesday, November 1, 2022.
Enclosed is the Informed Consent for Influenza Immunization form that is required for your child to receive the influenza vaccine. The two-page form must be filled out in its entirety for your child to receive their vaccine. Please return the form to school by October 28, 2022.
